What Exactly Is an FS-1 Form in North Carolina?
Hey neighbor — let's clear up the biggest myth first. The FS-1 is not a piece of paper you print and mail in. It's an electronic Certificate of Liability Insurance that your insurance company sends straight to the NC Division of Motor Vehicles.
It proves you had continuous liability coverage on the exact date the DMV is questioning. NCDMV only accepts it electronically from a company licensed to do business in North Carolina — printed copies, out-of-state policies, and screenshots won't cut it.
The FS-1 captures the year, make, model, and Vehicle Identification Number (VIN) of your insured vehicle, plus the policy effective dates. Once NCDMV receives it showing continuous coverage, the lapse notice is cleared and any pending fines drop off.
The FS-1 is a digital handshake between your insurer and NCDMV. You can't submit it yourself — only your NC-licensed insurance company can.
2026 update you need to know: Since July 1, 2025, every NC auto policy must carry minimum 50/100/50 liability limits ($50K per person bodily injury, $100K per accident, $50K property damage), plus mandatory uninsured/underinsured motorist (UM/UIM) coverage. If your policy meets these new limits, your FS-1 will clear the notice without issue.
